New full-time students must submit a completed Health History Form and provide evidence of having received the following:

2 doses of the Measles/Mumps/Rubella (MMR) vaccine after the first birthday

1 dose of the Meningitis vaccine for students under age 22

1 Tetanus/Diphtheria (Td) or Tetanus/Diphtheria/Pertussis (Tdap) vaccine booster within the last 10 years

3 doses of the Hepatitis B vaccine

Full-time students are also required to complete the Tuberculosis (TB) Risk Assessment form.

Students may also be required to show proof of a recent PPD Tuberculosis Skin test or IGRA blood test based on risk factors for TB.

This form must be returned by US mail or uploaded through the Monarch Wellness Portal byÌýAugust 1Ìýfor full-time students entering in the Fall orÌýJanuary 4thÌýfor full-time students entering in the Spring.
If this completed information is not received, a hold will block the student's registration for the second semester. If you prefer to submit the completed form by US mail, the form can be downloaded below. The mailing address is on the form.
Remember to keep a copy of your immunization forms for your records if you are mailing the form.ÌýDo not fax forms.ÌýThey must be uploaded through theÌýÌýor mailed.
